This policy explains what the Employers Preferred Choice (EPC) website collects, why, and your choices. The website collects business contact details only. It does not collect health information. Effective date to be set at launch.
What we collect
- Details you type into our forms: name, work email, phone, company or agency, NPN, states, group size and your message.
- Basic technical data from your browser, such as pages visited and referring site.
What we don't collect
Health information. Census files, claims and anything medical go only to separate protected services covered by a business associate agreement, never this website.
How we use it
- To reply to you and prepare an analysis or appointment you asked for.
- To understand which pages help people, in aggregate.
- We don't sell your information.
Where it goes
Form submissions are stored in EPC's business systems and seen only by the EPC team that responds.
Your choices
Ask us to see, correct or delete your contact details by emailing sales@epchealthplan.com.
